Text box

(optional) click on the text box and then click  on the desired place over the template. (For small string characters)

(optional) click on text area then click  on the desired place over the template.. (For large string characters)

Image box

(optional) Field to add image in the form.

Multiple values

Check box

(optional) Users can select multiple values in the form. The value and label for each of the options needs to be defined. 

Single value

Radio button

(optional) Users can select any one value out of the given values in the form. The value and label for each of the options needs to be defined.
