How to set up a form on digio?

  1. Creating a form:

  1. Click on Create form - If there is no underlying document that needs to be populated using the form

  2. Click on Create form with document template - If there is an underlying document template uploaded on DIGIDoc.



  1. Enter form name. 




  1. Adding fields in your form:

    1. Select Input type from the drop down

      1. Dropdown values:


Header

A title on the top of the form can be applied as a header. 

Text Box

A small placeholder for specific inputs. Eg: Name, Mobile Number, PAN No, etc.

Text Area

A large placeholder for multiline response

Drop Down

A list of pre populated values for user to select

Multiple choice

A multi select pre populated values for user to select

Single choice

A single select pre populated values for user to select

Date picker

A placeholder for taking date value . eg: Date of birth, date of application, etc.

File upload

Upload a file to the form. 


  1. Field Validation:


Editable

Defines that field can be modified by user

Hidden

Defines that the field would not be visible to user

Optional

Defines that the field will be optional for user to enter response

Email

Defines that value of the field has to be an email address

Mobile

Defines that value of the field has to be a mobile number

Numeric

Defines that value of the field has to be only a mobile number




  1. Finalize your form : Click on create form to finalize your form.


(b) Create form from document template

  1. Click on create form from document template

  2. Select the template you want to build the form with. 

  3. Enter form name and click on create form.

  4. Add fields in the form. Refer above steps from create form.

  5. Click on save form.
